With MyChart You Have The Ability To:
- See your lab and test results.
- Ask non-urgent questions to your Provider Care Team. You can send a picture or document, too.
- Schedule, view, or cancel an appointment with your PCP or a Specialist.
- Easily add your appointments to your calendar. All it takes is three clicks to add your connected calendar.
- Speed up the Check-in process. Use the precheck-in feature to answer questions and set up your co-pay.
- Request prescription renewals from your provider's office.
- Upload, request, and receive forms.
- Return to work, camp physicals, immunizations, and more via MyChart.
- Update your information.
- Make changes to your address, phone, email, insurance, and emergency contact person.
Don't Have MyChart Yet?
Call 508-992-6553, Monday through Friday, 8:00 am to 5:00 pm, to receive an activation code to sign up for your MyChart Account.
How To Get MyChart
- Provide your email address and request an activation email to be sent to you. Anyone at your provider’s office or central registration at 508-992-6553 can send this to you.
- From the activation email create your myChart account by entering:
- at least a 6-character username (case insensitive)
- at least an 8-character password (upper and lower case with numbers and letters, case sensitive)
- your date of birth
- Get the benefits of direct access to your clinical information.
Use the MyChart Mobile App
- Download the MyChart mobile application from the Google Play Store or Apple App Store. Enter "MyChart" in the search field. Select the icon and then click on the Get button.
- Find and select the Community Health Connections logo from the list of organizations.
- We recommend setting up TouchID or a Passcode for using the MyChart mobile app.
