Do You Need Health Insurance?
We Can Help.

If you would like to apply for health insurance coverage, you should speak with one of our benefits counselors before your visit with your medical provider.

Our benefits counselors can help you with the application process and determine your eligibility for affordable health insurance plans to make sure that you and your family stays healthy.

You will need to bring the following information with you when you visit a Health Benefits Counselor:

  • Photo ID (driver’s license, passport etc.)
  • Proof of income (two weekly pay stubs or one bi-weekly pay stubs, income tax forms)
  • Proof of Immigration Status (legal permanent resident card, visa, asylum, etc.)
  • Proof of Citizenship (a photo ID along with a birth certificate or a passport)

No one will be denied access to services due to an inability to pay. In addition, you may qualify for health coverage regardless of your immigration status.

Many private insurance plans are also accepted at the Health Center, and the list is growing every day. See the current list in the box on the right for medical and dental coverage.
