An Innovative Program Focused on Improving Outcomes

A Patient Centered Medical Home (PCMH) ensures timely access to comprehensive, patient-centered care by a team of primary care providers and their staff, who coordinate all aspects of a patient’s health care needs.

The patient is treated as a "whole person" with respect, dignity, and compassion. It’s a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits the patient’s needs.

Greater New Bedford Community Health Center has been involved in the Massachusetts Patient Centered Medical Home Initiative since 2011. Currently, our Pediatric department has completed the transformation to PCMH using the National Committee for Quality Assurance (NCQA) model as an approach to improve quality and efficiency.

In support of our effort to provide continuous care, a focus has been placed on caring for the patients between visits. Nursing and Medical Assistant staff have been integrated into the primary care practices and will outreach to patients to assure that they receive high quality, optimal care.
